Please fill this form out completely to make the boarding experience more pleasant for all involved. We do our best to make your pet(s) feel at home, including exercise outside several times a days and lots of TLC! Feel free to visit our kennel area.

** We routinely treat all animals for external and internal parasites as necessary. If your pet develops loose stool secondary to diet change and excitability, this will be medically managed at your expense. Be sure they are up-to-date on all vaccinations and are free of parasites. As we are sure you understand, this is for the protection of all other animals here. If your pet received vaccinations elsewhere, please provide proof of those vaccines and the dates they were given.

** Please provide written instructions for feeding and medicating (if applicable). Be certain that ALL medications are in properly labeled and in their original containers.

 NOTE: there will be an additional fee of $2.63 per day for medicating your pet during his/her stay.
